Transcript Requests

Please fill out and submit the online Transcript Request Form (below) for scholarships or requests other than initial college application requests.

If you are a senior needing to send an initial transcript to a college/university, please reach out to counseling and speak with one of the college counselors.

Seniors – end-of-year transcript requests will be available in May.  The college counselors will have a QR form for you to access the final transcript form.

Your request will be processed as quickly as possible. If you have any problems submitting the form, please call Christian Brothers at 916-733-3600.
